RO-DBT Therapy in Pittsburgh, PA
Brian Nuckols, MA, LAPC · Pittsburgh, PA
Brian Nuckols, LAPC, offers Radically Open DBT (RO-DBT) therapy and skills groups in Pittsburgh, PA. He works with adults whose core difficulty is not a lack of coping skills but an excess of control: people who regulate their emotions too tightly, hold themselves to punishing standards, and struggle to connect despite appearing competent to everyone around them.
What RO-DBT Treats
Radically Open DBT was developed by Thomas Lynch for a population that standard therapy consistently misidentifies. Most evidence-based treatments assume the patient needs more self-control. RO-DBT was designed for people who have too much of it. The clinical term is overcontrol, and it shows up as chronic rigidity, perfectionism, emotional constriction, compulsive need for structure, and social withdrawal that coexists with outward achievement.
RO-DBT has strong outcome data for treatment-resistant depression (the kind that does not respond to CBT or medication), anorexia nervosa, and obsessive-compulsive personality disorder. It also addresses chronic perfectionism, autistic burnout patterns, and any presentation where the problem is not what the person is doing wrong but how tightly they are holding on.
How RO-DBT Works
The central mechanism in RO-DBT is social signaling. Lynch’s research showed that overcontrolled people suppress the nonverbal signals (facial expression, tone, posture) that tell other people it is safe to connect with them. The result is loneliness that looks, from the outside, like self-sufficiency. Treatment focuses on increasing openness and flexibility in social signaling, not on acquiring more skills for managing emotions.
RO-DBT includes both individual therapy and a skills class. The skills class covers 30 lessons across five themes: radical openness, flexible mind, social signaling, awareness, and compassion. Brian facilitates RO-DBT-informed skills groups in Pittsburgh that address these themes in a group setting where participants practice new forms of social engagement with each other.
Who Benefits from RO-DBT in Pittsburgh
If you have completed courses of CBT, standard DBT, or medication trials without sustained improvement, and if the people in your life describe you as “fine” or “strong” while you feel isolated and exhausted, RO-DBT may be addressing the part of the problem that previous treatment missed. Brian sees adults in Pittsburgh presenting with anorexia and restrictive eating, chronic depression that has resisted standard treatment, perfectionism that has become debilitating, and the rigid interpersonal patterns that keep capable people from feeling connected.

Brian is in-network with Highmark and UPMC in Pennsylvania. He accepts VCAP (Victims Compensation Assistance Program) for eligible patients. For other insurance plans, he provides a superbill for out-of-network reimbursement. Private pay sessions are $150.
